  • Typically used for children with non-life-threatening allergies. It outlines how to manage mild to moderate allergic reactions and prevent exposure to allergens. Formal documents for managing specific medical conditions, particularly in emergencies or daily care.

    • Must be completed, signed by a medical practitioner, and include a color photo of the child.
    • Typically renewed every 12 months unless otherwise advised.
    • Essential for conditions with life-threatening implications or when ongoing medication is required.
